Cannabidiol and multi-modal exercise for cancer survivors suffering from chemotherapy-induced peripheral neuropathy

Background: Significant advances in cancer treatments have determined increased survival and higher rates of treatment-related sequela. Among these sequela, chemotherapy-induced peripheral neuropathy (CIPN) affects up to 20-40% of patients surviving cancer, who have been receiving taxane and/or platinum-based chemotherapy. Currently, all treatments available for CIPN provide limited benefits. Both cannabidiol (CBD) and multi-modal exercise (MME) have anti-inflammatory and analgesic potential and may provide protection/relief from CIPN. Previously, both interventions have been studied separately, and they have shown good potential to either prevent or treat CIPN in patients undergoing active cancer treatments. However, little is known regarding the effect of a combination of CBD and MME on patients surviving cancer and suffering from CIPN.Purpose: We conducted a four-month interventional open-label study to explore CBD’s effect with and without MME to improve CIPN signs and symptoms, quality of life (QoL) and functional capacity in patients who had completed cancer treatments.Methods: Study patients (n=27) underwent two consecutive two-month interventions, CBD (up to 300 mg/day) followed by CBD combined with MME. The painDETECT questionnaire was used to assess CIPN-related neuropathic pain. CIPN neurotoxic symptoms, perceived physical function and overall QoL were assessed via the Functional Assessment of Cancer Treatment/Gynecological Oncology-Neurotoxicity-13. Participants’ functional status was examined through gait speed and timed up and go for mobility, 9-hole peg test for manual dexterity, hand grip strength and five repetitions of sit-to-stand for dynamic balance and overall strength.Results: After two months of CBD treatment alone, moderate to large positive effect sizes were observed for CIPN neurotoxic symptoms, perceived physical function, and overall muscle strength. Following the combined interventions, large to very large positive effects were noted in CIPN neurotoxic symptoms and perceived physical function. In contrast, moderate to large effects were seen in QoL, gait speed, and manual dexterity.Conclusions: These findings offer proof of concept for the therapeutic potential of CBD, both as a standalone treatment and in combination with MME, in alleviating CIPN symptoms, enhancing QoL and reducing functional impairments among patients who have completed cancer treatments. Further randomized, placebo-controlled trials are warranted to validate these preliminary results. Considerations for future cannabinoid-based clinical studies are also discussed
