Relating Process to Outcome: Resolution of Self-Criticism Process, Client Post-Session and Post-Treatment Changes, and Working Alliance in Emotion-Focused Therapy for Depression

Self-criticism is widely recognized as a key factor in the onset and maintenance of depression. The two-chair technique in Emotion-Focused Therapy is one of the few therapeutic approaches specifically designed to address self-criticism. Despite the effectiveness of this technique in resolving self-criticism, a gap remains in understanding the mechanisms through which the resolution of self-criticism process unfolds. The validation phase of task analysis was used to quantitively explore the relationship between process and outcome in the self-criticism process. It tested whether the phases delineated in the previously developed Refined Performance Model of Conflict Resolution (RPMCR; Greenberg, 1980), which focuses on intrapersonal conflict with an emphasis on decisional conflict, apply to the resolution of self-criticism process. Moreover, the current study examined the influence of relational factors, such as the working alliance, on resolution of self-criticism process and post-session outcomes. The empirical phase of task analysis was also used to qualitatively examine the step-by-step of this process with particular focus on two hindering phases of collapse and impasse. Transcripts and videos of 27 clients engaged in the resolution of self-criticism process were selected from archival data (Watson et al., 2003) and rated using the Degree of Resolution Scale (DRS; Greenberg, 1984). All self-reported measures, including post-session and post-therapy outcomes, were extracted from the original study. The validation phase confirmed the RPMCR’s validity, revealing a significant correlation between the process and outcomes of resolution of self-criticism. Specifically, a higher degree of self-criticism resolution was associated with greater client change following sessions with resolution process, as well as lower levels of self-criticism at termination. Although no direct link was found between the therapeutic alliance and the resolution process, a stronger alliance correlated with improved client perceptions of change post-session. The empirical phase also validated the RPMCR and contributed to the development of a refined model called the Resolution of Negative Self-Treatment Process (RNSTP). This model elaborates on previous theories and models of intrapersonal conflict and outlines the nuance and details of steps in resolution of self-criticism process, with particular focus on collapse and impasse. Implications for future research and clinical practice are discussed.
