A Case of Using Delta-lock Stem for Breaking LCP Plate after Osteosynthesis of Periprosthetic Fracture after Total Hip Arthroplasty

Abstract
大腿骨ステム周囲骨折後のLCP curved broad plate（以下LCP）折損例に対してデルタ-LOCK stemを用いた1例を経験したので報告する．症例は80歳女性，71歳時右人工股関節全置換術（以下THA），revision THAの既往あり．自宅で転倒し右大腿骨ステム周囲骨折Vancouver分類type Cを受傷．LCPとcable wire systemを併用して観血的整復固定術を施行．術後12週目に歩行練習中に誘因なく右下肢に脱力感が出現し，LCP折損を認めた．デルタ-LOCK stemを用いてrerevision THAを施行した．本症例は高齢者の元々の骨脆弱性に加え，セメント周囲の骨折，横骨折であり，骨癒合が得づらい条件であった．また，遷延癒合を認める状態での荷重や骨折部の僅かなギャップがLCPの疲労折損の原因になったと推測される．このように骨癒合が得づらい症例ではplate折損等の合併症を含めたインフォームドコンセントが必要であると考える．
