Abstract
Problemstellung: Im Phantomversuch wird die Korrelation zwischen Dopplergeschwindigkeit, eingestellten Volumenfluss und der Zeitintensitätskurve ermittelt Methoden: UHCD Flusspumpe (Shelly Medical Imaging, Ontario, Canada) mit Blutersatzflüssigkeit (Partikelgröße ca.10µ). Phantom: Wasser/Mehl/Gelantine. Flow Setting: Arterielles Niedrigwiderstandsflussprofil mit einer Vmaxdia/sys von 11–129cm/s bei Flussvolumina zwischen 5mL und 25mL. US Gerät: GE LOGIQ 7m TX 7MHz, LA (GE Healthcare Systems, Solingen). Acoustic output (AO): 10–100%. Silikonschlauch (10mm). Alle Bildsequenzen wurden als Rohdaten digital abgespeichert und konnten so nachbearbeitet werden. Dopplersonographische Bestimmung der Vmaxsys,Vmaxdia, Vmean bei einem Winkel <45Grad. Im THI mode wird der Schlauch im Querschnitt eingestellt und aus der an das Lumen angepassten ROI die time intensity curve (TIC) bestimmt. Die TIC wird an der systolischen und enddiastolischen Dopplerspitzengeschwindigkeit kalibiert und die area under the curve für mehrere Pulszyklen berechnet. Die Signalintensitäten steigen mit zunehmendem AO. Das Produkt aus AUC und Lumenfläche wird mit den eingestellten Volumenflüssen korreliert. Ergebnisse: Limitationen bei der Auswertung waren Bewegungsartefakte des Schlauchs bei hohen Flussvolumina und blooming bei hohen akustischen Ausgangsleistungen. Korrelation zwischen AUC und mittlerer Dopplergeschwindigkeit 0.98 und 0.99. Korrelation zwischen Volumenfluss berechnet aus der AUC x Fläche und dem eingestellten Volumenfluss lag zwischen r=0.92 und r=0.99. Der Volumenfluss berechnet aus den Rückstreuechos ist nach Kalibrierung unabhängig von der akustischen Ausgangsleistung des Gerätes. Schlussfolgerungen: Mithilfe einer Doppleranalyse gelingt es den Signalstärkenumfang der Rückstreuechos der TIC an einer Geschwindigkeitsskala zu kalibrieren. Die aus der Querschnittsfläche berechneten Intensitätswerte der Rückstreuechos über die Zeit gestatten eine zuverlässige Bestimmung des Volumenflusses.
