Abstract
La prevention et la promotion de la sante font partie integrante des soins primaires. En France, les medecins generalistes en sont les principaux acteurs. L'application des recommandations en matiere de prevention est souvent jugee insuffisante. Le manque de temps et la divergence des recommandations pourraient en etre en partie responsables. Une premiere etude a mis en evidence la difficulte a comparer les recommandations americaines, canadiennes et francaises en matiere de prevention chez l'adulte. Leur degre d'accord etait faible, mais dans une moindre mesure lorsque les recommandations concernaient le recueil d'informations par l'interrogatoire et l'examen physique. Les recommandations les plus concordantes identifiees pourraient guider le recueil des facteurs de risque en medecine generale. Le projet ECOGEN a ete mene en France entre novembre 2011 et avril 2012. Cette vaste etude observationnelle, transversale, multicentrique, reposait sur un recueil des donnees de consultations par des observateurs externes. Dans le cadre de ce projet, une etude ancillaire a evalue l'activite preventive des medecins generalistes francais : 14,9 % des consultations non dediees initialement a la prevention. Celle-ci dependait moins des caracteristiques mesurees du medecin generaliste, que de l'âge de sa patientele et de l'organisation de son activite. La duree de la consultation etait un determinant majeur de la delivrance de soins preventifs, alors que le paiement a l'acte prevaut en medecine generale. Cette these apporte des elements pertinents en faveur de l'amelioration des strategies preventives chez l'adulte en medecine generale, tant a court terme qu'a plus long terme.
