Abstract
1病例资料男,65岁,主因上腹部不适1年余,发现胃多发息肉半年入院。患者1年前无明显诱因出现上腹部不适,无发热、疼痛及腹泻,间断自行口服药物治疗（具体不详）,疗效欠佳。半年前在外院行胃镜检查提示胃多发息肉,为切除胃息肉入我院。查体：头发稀少,双手指甲扁平,失去光泽,大拇指指甲开裂,双肩部以及腰部、双足底有明显色素沉着（图1、2）。心肺检查无异常。腹部查体无异常。家族中无类似病例。实验室检查：血红细胞3.75＆#215;1012/ L,血红蛋白130 g/ L,白细胞3.83＆#215;109/ L。红细胞平均体积99.70 fL,平均血红蛋白含量34.70 pg,单核细胞比率15.3℅。生化检查：血肌酐47μmol/ L,血尿素氮1.97 mmol/ L,钙1.99 mmol/ L,磷0.75 mmol/ L,钾3.7 mmol/ L,钠133 mmol/ L,总蛋白43.5 g/ L,白蛋白31.0 g/ L,球蛋白22.5 g/ L。粪便常规：隐血（-）。血清铁代谢：总铁结合力46.0 μmol/ L,血清铁蛋白29.2 pg/ L,转铁蛋白1510 mg/ L。抗核抗体阴性,抗 ds-DNA 抗体阴性,抗 sm 抗体阴性。 IgE 抗体阳性。胃镜：胃底、胃体黏膜广泛充血水肿糜烂、明显增厚,并可见散在黑色素沉着,胃角、胃窦多发息肉,结节状隆起,表面充血糜烂（图3、4）。超声内镜检查提示胃底、体黏膜明显增厚。病理提示：潴留性息肉,黏膜组织内可见较多嗜酸细胞浸润,部分腺体明显扩张（图5）。结肠镜检查：降结肠息肉。病理：潴留性息肉,黏膜组织间可见嗜酸细胞浸润。结 合 患 者 临 床 症 状,符合Cronkhite-Canada 综合征（CCS）诊断。给予胃肠道息肉摘除,并对症治疗,病情稳定后出院。术后2个月随访,患者病情稳定,胃肠镜以及超声内镜复查较出院时无明显变化,继续给予定期随访。
