Abstract
Resume Objectif : Evaluer la pertinence et l’applicabilite du systeme de classification anatomique, therapeutique et chimique et dose definie journaliere pour le suivi de l’utilisation des antibiotiques dans les etablissements de sante quebecois. Mise en contexte : L’augmentation des cas des diarrhees associees au Clostridium difficile dans les hopitaux quebecois depuis 2003 a suscite le developpement de nouvelles lignes directrices par le Comite des infections nosocomiales du Quebec en 2005. Une des mesures pour pallier ce probleme est d’optimiser l’utilisation appropriee des antibiotiques. Afin de surveiller l’utilisation des antibiotiques dans les etablissements de sante quebecois, il est maintenant recommande d’utiliser le systeme de classification anatomique, therapeutique et chimique et de dose journaliere definie. Methode : Une revue de la documentation a ete effectuee concernant les differentes methodes de mesure de la consommation des antibiotiques a partir des bases de donnees Medline et International Pharmaceutical Abstracts. Conclusion : Le systeme de classification anatomique, therapeutique et chimique et de dose journaliere definie est applicable comme indicateur de premiere ligne pour la comparaison a grande echelle de la consommation des medicaments. Des methodes plus precises peuvent s’averer necessaires afin de verifier les observations initiales. Le recours a ce systeme dans le cadre d’activites d’evaluation de l’utilisation des medicaments au Quebec devrait etre encourage. Abstract Objective: To evaluate the relevance and applicability of the anatomical, therapeutic and chemical and defined daily dose classification system used to document antibiotic use in Quebec healthcare institutions. Context: The increase in the number of cases of Clostridium difficile associated diarrhea in Quebec hospitals since 2003 has prompted the development of new guidelines in 2005 by the Comite des infections nosocomiales du Quebec. One way to resolve this problem is to optimize appropriate antibiotic use. In order to monitor antibiotic use in Quebec healthcare institutions, it is now recommended to use the anatomical, therapeutic and chemical and defined daily dose classification system. Methods: A literature review was conducted for the different methods of assessing antibiotic use using the Medline and International Pharmaceutical Abstracts search databases. Conclusion: The anatomical, therapeutic and chemical and defined daily dose classification system is applicable as a primary indicator in the comparison of large-scale medication use. More precise methods may be required to confirm initial observations. Recourse to this system for drug use evaluation in Quebec should be encouraged.
