Abstract
Overexpression of P-Glycoprotein (Pgp) was linked to multidrug resistance (MDR) in mammalian cell lines and human cancers, evoking intense interest first from molecular and cell biologists and later, when purified Pgp became available, from biochemists and biophysicists. The transport activity of Pgp has been studied in both intact cells and simpler subcellular systems, such as plasma membrane vesicles and reconstituted proteoliposomes. The vacuum cleaner and flippase models propose that drugs and modulators gain access to the Pgp binding pocket through the lipid bilayer. Pgp plays a central role in the absorption and distribution of many drugs used to treat human diseases and is an important determinant of their pharmacokinetic profile and, ultimately, the clinical response. Many human cancers become simultaneously resistant to several different anticancer drugs, leading to failure of chemotherapy treatment. Pgp polymorphisms can alter the pharmacokinetics of its substrates and thus influence the clinical outcome of drug treatment.
