Lorsqu'il fut question, en 1976, d'implanter dans le centre-ville de Montréal un réseau de services et d'activités destiné aux personnes âgées du quartier, les réactions furent rapides et désarmantes. Des personnes, connaissant la population du centre-ville, concluaient déjà sans nuance : « Le quartier est trop détérioré. Les gens du centre-ville sont des marginaux. Aucune intervention n'est possible dans un tel milieu. Mieux vaut référer les gens à l'Accueil Bonneau ou à l'Armée du Salut ! » Au lieu de nous démobiliser, ces commentaires empreints pourtant d'impuissance, de scepticisme et de résignation, nous stimulèrent parce qu'ils nous provoquaient à relever le défi. Ce défi sous-tendait les problèmes suivants : Comment implanter des services adaptés à la mentalité et aux besoins des personnes âgées ? Comment développer des conditions de vie communautaire favorables à la prévention sanitaire et sociale ? Dans un quartier aussi détérioré, comment orienter notre travail sur la promotion de la santé plutôt que sur le traitement de la maladie, comme le requiert le mandat dévolu au CLSC ? Comment privilégier une approche de la personne plutôt globale que parcellaire ou spécialisée, c'est-à-dire une approche où toutes les dimensions de la santé seraient en interrelation les unes avec les autres, où les diagnostics établis et les traitements prescrits le seraient à la lumière des caractéristiques économiques, sociales, et psychologiques des individus ?
