Evaluation of Arthroscopic Capsular Release Together with Manipulation Under Anesthesia for Treatment of Frozen Shoulder

Abstract
Amaç: Bu çalışmada, anestezi altında manipülasyon (AAM) ile beraber artroskopik kapsüler gevşetme yapılan hastalarda uzun dönem sonuçları değerlendirilmiştir. Aynı zamanda ek patolojisi olan hastalarla olmayanlar arasında, fonksiyonel açıdan ve yaşam kalitesi yönünden herhangi bir farkın olup olmadığı da araştırılmıştır. Gereç ve Yöntemler: 2003-2008 tarihleri arasında AAM ile birlikte artroskopik kapsüler gevşetme yapılan 20 hasta değerlendirmeye alındı. Hastaların 16 tanesi kadın, dört tanesi erkek, ortalama yaş 55.75 (44-77) ve ortalama izlem süresi 41.60 (12-72) ay olarak belirlenmiştir. Hastaların ameliyat öncesi klinik değerlendirmeleri Constant skoru, ağırlıksız Constant skoru, Western ontario rotator manşet indeksi (WORC), eklem hareket genişlikleri, izometrik kas gücü ölçümleri ile yapılmıştır. Bulgular: Hastaların öne fleksiyon ve abdüksiyon açısı sırasıyla ortalama 50.50-50 dereceden 162.25-165.75 dereceye çıkmıştır (p< 0.001). Doksan derece abdüksiyonda eksternal rotasyon ve 0 derece abdüksiyonda eksternal rotasyon derecelerine baktığımızda sağlam tarafa göre sırasıyla %97'lik ve %89'luk bir kazanım elde edilmiştir. Operasyon öncesi ortalama 14.75 olan Constant skorlaması, postoperatif izlemde ortalama 74.89 olarak saptanmıştır (p< 0.001). Ağırlıksız Constant skorlaması operasyon öncesi ortalama 14.75 puandan operasyon sonrası ortalama 68.25 puana çıkmıştır (p< 0.001). Sağlam tarafı yakalama yüzdesine göre, abdüksiyon kas gücünde %107, eksternal rotasyon kas gücünde %86 oranında bir kazanım elde edilmiştir. WORC indeksi operasyon öncesi ortalaması 1758 puandan, operasyon sonrası ortalaması 248 puana inmiştir (p< 0.001). Ek hastalığı olan grup ile olmayan grup arasında, fonksiyonel açıdan ve yaşam kalitesi yönünden bir fark saptanmamıştır (p> 0.05). Sonuç: Konservatif tedavinin başarısız olduğu durumlarda, tedavi süresini kısaltmak için AAM ile birlikte artroskopik kapsüler gevşetme, uzun dönemde omuz fonksiyonunu ve yaşam kalitesini arttıran bir yöntemdir.
