The Significance of Thrombin Generation Test in Recurrent Early Pregnancy Loss

Abstract
Amaç: Bu çalışmada, tekrarlayan abortus (TA) tanısı konulan, trombofili dışında patoloji saptanmayan hastalarda trombin üretim ölçümü [Thrombin Generation Assay (TGA)] testinin kullanımını araştırmayı amaçladık. Gereç ve Yöntemler: Erciyes Üniversitesi (ERÜ) Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı'na başvuran ve TA tanısı konulan 100 hasta ve 25 sağlıklı, tekrarlayan düşükleri olmayan kadın olmak üzere toplam 125 kadın çalışmaya alınmıştır. Çalışmaya olgu grubunda alınan kişiler trombofili sonuçlarına göre trombofili pozitif ve negatif olanlar olmak üzere ikiye ayrılmışlardır. Kontrol grubuna alınanlar ise rastgele örnekleme ile sağlıklı doğum yapmış kadınlardan seçilmiştir. Her iki gruptaki kadınlardan trombofili faktörleri ve koagulasyonu değerlendirmede tanımlanmış bir test olan TGA için kan örnekleri alınmış ve ERÜ Hematoloji ve Genetik Laboratuvarları'nda analiz yapılmıştır. Bulgular: Araştırmamıza trombofili pozitif olan 51 hasta, trombofili negatif 49 hasta dâhil edilmiş olup, kontrol grubundaki 25 kişinin de 2'sinde trombofili saptanmıştır. TGA testinde en değerli parametre olan endojen trombin potansiyeli (ETP) değeri trombofili pozitif grupta en yüksek değere sahipken, trombofili negatif grup ve kontrol grubu arasında istatistiksel olarak anlamlı fark bulunmamıştır. Duraklama zamanı, pik zamanı ve kuyruk başlangıcı, TA'sı olan hastalarda kontrol grubuna göre istatistiksel olarak anlamlı düzeyde daha uzun bulunmuştur. TGA'nın, trombofili belirteçlerinden bağımsız olarak %96 (%95 güven aralığı-GA 86,5-99,4) duyarlılık ve %96 (%95 GA 79,6-99,3) özgüllük ile trombofili pozitifliği bulunan TA'lı hastaları saptayabildiği gösterilmiştir. Sonuç: Son yıllarda koagülasyonu değerlendirmede kullanılan TGA testinin TA'sı olan hastaların taranmasında etkili, hassas ve diğer testlerden bağımsız olarak kullanılabileceği, literatürde ilk defa bu çalışmada gösterilmiştir.
