Objetivo: El objetivo de este estudio prospectivo ha sido presentar los resultados terapeuticos a largo plazo (protesicos e implantarios) de los pacientes desdentados tratados con protesis fija soportada por implantes, que participaron en el primer estudio clinico de implantes de Norteamerica. Material y metodos: Se trataron 45 pacientes con implantes Branemark que sostenian un total de 47 protesis fijas (42 inferiores y 5 superiores), entre 1979 y 1984. Todos los pacientes fueron revisados de forma regular para realizar evaluaciones prospectivas exhaustivas, de tipo clinico y radiografico. Resultados: Acudieron a la cita final de revision, en 2002, 31 pacientes (33 protesis), con unas tasas de exito de los resultados del plan protesico global y de los implantes del 84 y el 87%, respectivamente. La perdida media de hueso marginal alrededor de los implantes, despues del primer ano de carga fue pequena (0, 05 mm/ano), con grandes variaciones individuales. La escasa higiene oral, antecedentes de fumar tabaco y la posicion de los implantes parecian ser predictores de la perdida de hueso marginal. Se continuo el mantenimiento protesico, e incluyo los componentes fracturados y la sustitucion de las protesis. La vida de una protesis fija para este grupo de pacientes fue de 8, 39 ± 5, 30 anos. Conclusiones: Este estudio ha confirmado el exito global del resultado terapeutico a largo plazo de los pacientes tratados con protesis fijas soportadas por implantes Branemark. El exito de la osteointegracion con una pequena perdida media de hueso se mantuvo al envejecer los pacientes, aunque se requirio de un mantenimiento protesico. Esta ultima consideracion deberia comentarse con todos los pacientes que buscan dicho tratamiento.
