The Effect of Psychopathology on the Quality of Life and the Disability of Patients with Chronic Neck Pain

Abstract
Amaç: Çalışmamızda, süreğen boyun ağrısıyla başvuran hastalarda yetiyitimi ile depresyon ve anksiyete düzeyleri, aleksitimi ve somatik duyumları abartma ilişkisinin araştırılması amaçlandı. Gereç ve Yöntemler: Çalışmaya nöroşirurji polikliniğine süreğen boyun ağrısı yakınmasıyla başvuran 178 hasta dâhil edildi. Katılımcılar sosyodemografik veri formu, Görsel Analog Skala (GAS), Boyun Özür Göstergesi (BÖG), Beck Anksiyete Envanteri (BAE), Beck Depresyon Envanteri (BDE), Bedensel Duyumları Abartma Ölçeği (BDAÖ), Toronto Aleksitimi Ölçeği (TAÖ) ve Kısa form-36 (SF-36) ile değerlendirildi. Bulgular: Çalışmamızda ağrı şiddeti ile boyun özür düzeyi arasında, pozitif yönde zayıf-orta derecede korelasyon vardı. Depresyon ve anksiyete düzeyleri ile boyun özür düzeyi arasında da pozitif yönde zayıf-orta derecede korelasyon saptandı. Yaşam kalitesi ile boyun özür, depresyon ve anksiyete düzeyleri arasında negatif yönde korelasyonlar vardı. Ağrı şiddeti, depresyon ve anksiyete düzeyinin, boyun özür düzeyinin yordayıcıları olduğu tespit edildi. Beklenenin aksine, lineer regresyon modelinde süreğen ağrılı hastalarda, aleksitimi pozitif yönde yordayıcı olarak saptanmadı. Sonuç: Boyun ağrısıyla başvuran hastalarda biyopsikososyal yaklaşım göz önünde bulundurulmalıdır. Ağrıyla başvuran hastalarda kapsamlı bir psikiyatrik muayenenin, hatta yapılandırılmış psikiyatrik ölçeklerle değerlendirilmenin önemli olduğunu düşünmekteyiz.
