Validation of a commercial Gluceptate kit for in vitro Tc-99m labeling of red blood cells

1649 Objectives In vivo Tc-99m labeling of red blood cells(RBC) is primarily used for gated blood pool studies but can suffer from low labeling efficiences(LE)/drug interferences. In vitro labeling provides higher LE needed for gastrointestinal bleed, splenic imaging, hepatic hemangioma and blood volume determinations but clinical reimbursements rarely cover cost differentials for unit dose in vitro labeling kits. We set out to validate a cost effective multidose in vitro labelling technique using Health Canada licensed products. Methods Draximage Gluceptate kit (25 mg calcium gluceptate, 3 mg SnCl2.2H2O) undergoes aseptic double dilution resulting in 7.5 ug SnCl2.2H2O in 0.4 ml saline. 5 ml heparinized whole blood is added/mixed for 5 min. A 4 ml saline rinse of sterile vacutainer with 10.6 mg K2EDTA is added to chelate extracellular tin prior to 3 min centrifugation (stopper end down at 1000g). 2 ml packed RBC are added to 1000 MBq of pertechnetate and incubated 7 min. LE is determined by washing 0.2 ml labeled cells with 3 ml of saline. Plasma stability of 1 ml labeled RBC mixed with 12 ml plasma is assessed at 0, 1, 2, 4, 6 & 24hr. Results LE was 98.1+ 1.1% , n=187 patients, 20 staff. First milking of generators did not adversely impact LE despite build up of Tc-99g. Prefilling syringe hubs with saline avoids oxygen addition. High LE (97.6 + 0.7%,n=8), sterility and apyrogenicity was maintained when used for multiple patients up to 7 hr. Stability testing in plasma (n=16) was excellent: 97.2%(1hr), 96.2%(2hr), 93.9%(4 hr)& 92.2%(6hr). Conclusions Careful aseptic double dilution of a commercial stannous gluceptate kit and dilute EDTA rinse resulted in in vitro Tc-99m LE over 97% with excellent plasma stability.
