Fraturas periprotésicas em artroplastias da anca: Estratégias cirúrgicas

Abstract
As fraturas periprotésicas em artroplastia da anca são um tipo de complicação que podem exigir uma solução cirúrgica difícil. O objetivo deste estudo foi avaliar a eficácia de algumas estratégias cirúrgicas nos diferentes tipos de fraturas. As fraturas femorais periprotésicas foram classificadas de acordo com uma Classificação de Vancouver modificada. Tratámos 112 doentes (67 homens e 45 mulheres), 105 fraturas femorais (94%) e 7 fraturas acetabulares (6%). O follow-up médio foi 10,6 anos. Nas fraturas dos Tipos A (7 casos-7%), B1 (10 casos-10%) ou C (13 casos-12 %) com hastes femorais bem fixadas e estabilizadas apenas fixámos as fraturas. Nas fraturas dos tipos B2 (17 cases-16%), B3 (46 casos-44%) e D (12 casos-11%, casos clinicos onde a fratura envolvia 2 ou mais níveis femorais, situação não prevista na Classificação de Vancouver) utilizámos hastes femorais longas não cimentadas, fixação das fraturas com cabos metálicos e enxerto esponjoso alógeno para preenchimento das perdas ósseas femorais. Registámos como complicações 3 casos de infeção (2,7%), 3 luxações precoces tratadas com reduções fechadas, 2 casos de pseudartroses trocantéricas assintomáticas e uma refractura femoral. Necessitámos de reoperar 4 doentes (3,6%): 3 casos de infeção (2 casos de revisão em 2 tempos e 1 casos de remoção dos componentes) e 1 caso de refractura do fémur (reosteossíntese de fratura femoral). No tratamento de fraturas periprotésicas em artroplastias da anca é importante ter uma filosofia e estratégia de tratamento e disponibilidade do material cirúrgico mais adequado à resolução de cada caso clínico.
