Abstract
A 12-year-old previously healthy boy presented to the hospital with a three-week history of fever. The fever was initially associated with headaches that woke him up at night. He also reported having night sweats, anorexia and lethargy. He was prescribed an antibiotic (cefaclor) by his primary care physician for an erythematous oropharynx, although the swab taken had a negative bacterial culture. He denied having a cough, rash, diarrhea or dysuria. There was no recent travel, visitors from abroad or exposure to sick contacts. On examination, he was febrile at 39.2°C, with a heart rate of 90 beats/min and a blood pressure of 132/52 mmHg. A grade II/VI systolic ejection murmur was heard over the left upper sternal border without radiation. He was tender at the metatarsophalangeal joints of his left foot, without associated warmth or limited range of motion. There were no swollen joints, lymphadenopathy or rash. Investigations revealed a hemoglobin level of 96 g/L (normal 120 g/L to 160 g/L) with a mean corpuscular volume of 79.0 fL (normal 77.0 fL to 96.0 fL), erythrocyte sedimentation rate of 17 mm/h initially, 100 mm/h on repeat (normal lower than 10 mm/h) and a C-reactive protein level of 193.1 mg/L (normal lower than 8 mg/L). His liver enzymes and function, and creatine phosphokinase levels were normal, as were his chest x-ray and electrocardiogram.
