Une série de preuves scientifiques indique maintenant que les réponses placebo ou nocebo ne sont pas simplement des réinterprétations psychologiques de l’activité nociceptive. En fait, les réponses placebo et nocebo produisent des effets physiologiques bien réels et mesurables. Parmi les facteurs psychologiques qui déclenchent une réponse placebo ou nocebo, le conditionnement et les attentes affectent considérablement les résultats de la perception de la douleur ainsi que la réponse au traitement. Selon le contexte thérapeutique, les réponses placebo ou nocebo peuvent être déclenchées sans l’administration d’une substance inerte et auront une incidence significative sur les résultats du traitement. Dans cet article, nous allons décrire différentes situations où les facteurs psychologiques produisent des changements physiologiques du signal nociceptif à tous les niveaux du système nerveux central (SNC). Finalement, nous discuterons de l’importance de bien comprendre les mécanismes du placebo et du nocebo pour en tirer le meilleur parti dans un contexte clinique.
