Une revue systematique de 11 guides internationaux montre qu’ils ne sont pas tous en accord avec les donnees factuelles disponibles… Ce travail realise sur fonds public de l’Agency for Healthcare Research and Quality americaine avait pour objectif de verifier si les guidelines (tous anglophones) sur les antidiabetiques oraux tenaient reellement compte des donnees factuelles et a quel point leur qualite d’elaboration intervenait. Sur les 11 guidelines repondant aux criteres de qualite predefinis, 7 privilegiaient le choix en premiere ligne de la metformine, 10 signalaient que les thiazolidinediones etaient associees a une augmentation du risque d’oedeme et d’insuffisance cardiaque congestive comparativement aux autres antidiabetiques oraux, 1 ne donnait aucun niveau de preuves et seulement 5 avaient les 7 memes conclusions. Les scores evaluant la rigueur d’elaboration du guide et l'independance editoriale variaient considerablement, respectivement de 17 a 100 % (mediane 29 %) et de 8 a 100 % (mediane 75 %). Que retenir pour notre pratique ? • Que cette analyse ne porte que sur des textes anglophones ne change rien a l’affaire. Il n’existe que peu de « recommandations » indiscutables et toutes doivent etre passees au crible d’une lecture attentive et critique. Il n’existe pas de « cahier de recettes » universel du bon medecin… • A plus forte raison, il n’y a pas de relation entre le respect de guides apparemment pret-a-porter et la qualite des soins : l’allusion aux actuels CAPI francais n’est pas fortuite. • Dans le « peloton de tete » : le guide du NICE (2009) et ceux des 2 associations canadienne et americaine de diabetologie (CDA 2008, ADA 2009).
