L’etude longitudinale aupres de pres de 400 Quebecois souffrant de troubles du sommeil montre que l’insomnie est souvent chronique, en particulier quand elle atteint le seuil « significativement clinique » qui motive la consultation. Les sujets de l’enquete ont ete selectionnes par tirage au sort parmi les participants d’une enquete epidemiologique plus large menee au Quebec, contactes par telephone pour etre inclus dans un suivi de 3 ans avec questionnaires standardises effectues annuellement. Dans cette population, 74 % relatent une insomnie persistant depuis plus d’un an, pres de la moitie une insomnie persistant les 3 annees de l’etude (ce sont surtout ceux qui souffraient d’une insomnie severe au depart, les femmes et les plus âges). Les troubles moins severes etaient trois fois plus nombreux a disparaitre qu’a s’aggraver, mais meme dans ce groupe la chronicite etait frequente Morin CM, Belanger B, Le Blanc M, Ivers H, Savard J, Espie CA et al. The Natural History of Insomnia. Arch Intern Med. 2009;169:447-53. Les questions que se pose la redaction • Insomnie un jour, insomnie toujours ? L’etude quebecoise retrouve cette caracteristique chez les insomniaques « severes », comme dans quelques autres etudes longitudinales anterieures sur des durees allant jusqu’a 20 ans. • Une insomnie severe d’emblee merite en tout cas une prise en charge serieuse, puisqu’elle semble etre un reel facteur pronostique a long terme, malgre les multiples evolutions possibles. • L’etude ne permet pas – ce n’etait pas son objectif, qui etait seulement « l’histoire naturelle de l’insomnie » – d’analyser les eventuelles co-morbidites medicales ou psychiatriques, si importantes pour la prise en charge.
