Manejo clínico y nutricional de la falla renal aguda de origen medicamentoso en caninos. Reporte de caso.

Abstract
La falla renal aguda se define como un descenso abrupto y sostenido en la tasa de filtracion glomerular, que conduce a la acumulacion de productos nitrogenados y toxinas tales como urea y creatinina y cursa con oliguria o anuria, debido a necrosis tubular aguda (90% de los casos) asociada con isquemia renal, agentes nefrotoxicos o la combinacion de ambos. En el presente articulo se describe un caso de un canino labrador retriever de 2.5 anos de edad, que fue llevado al servicio de consulta externa del Centro de Medicina Veterinaria y Zootecnia de la Universidad CES (Medellin, Colombia) con vomito y diarrea persistentes de dos dias de evolucion, durante los que habia sido tratado con medicamentos para un diagnostico presuntivo de erlichiosis, incluyendo oxitetraciclina (20 mg/kg) intravenosa. La falla renal aguda se diagnostico mediante analisis integrado de los anamnesicos, el examen fisico general, el hemograma, la medicion de creatinina, potasio, fosforo y ALT sericos, y el citoquimico de orina. Al paciente se le instauro fluidoterapia con sodio cloruro y dextrosa al 5%, con ranitidina (2 mg/Kg) y metoclopramida (0.2 mg/kg) y dieta de prescripcion hiposodica, hipofosfatemica e hipoproteica. La remision de los sintomas se logro a los 5 dias de tratamiento, cuando la creatininemia se normalizo aunque el paciente permanecio con polidipsia y poliurea durante la fase de recuperacion; la azotemia, la poliuria y la polidipsia mejoraron y el paciente fue dado de alta con dieta hipoproteica e hiposodica, con una evolucion adecuada y una recuperacion completa. Abstract Acute renal failure is a common disease defined as the sudden and sustained decline of glomerular filtration rate, that leads to the accumulation of nitrogenous waste products and ureic toxics such us urea and creatinine, in patients suffering from acute tubular necrosis-related oliguria or anuria (90 % of cases) as a result of renal ischemia, renal toxics or both. In this paper a two and half year old Labrador retriever canine patient is described which was received at the “Servicio de consulta externa del Centro de Medicina Veterinaria y Zootecnia de la Universidad CES (Medellin, Colombia)” with a two day lasting vomiting and diarrhea. The patient was receiving a presumptive ehrlichiosis diagnosis-related medical treatment including intravenous Oxytetracyclin (20 mg/kg). Acute renal failure was diagnosed according to an integrated analysis of anamnesics, physical examination, hemoleukogram, creatinine, potassium, phosphorus and ALT serum levels, and urianalysis. The patient was treated with sodium chloride and 5% dextrose fluid therapy, ranitidine (2 mg/Kg), and methoclopramide (0.2 mg/kg) combined with a commercial protein restricted kidney formulation consisting of a hyposodic, hypophosphatemic and hypoproteic diet. Recovery of the symptoms was achieved after 5 days of treatment when creatinine serum levels reached the normal physiological values instead of a persistent polydipsia and poliurea during the recovery phase. Following the recovering of azothemia, poliurea, and polydipsia condition the hypoproteic and hyposodic prescription diet was recommended for another 45 more days, until the patient exhibited a complete recovery of its renal function.
