Abstract

Objectif : Notre étude visait à cerner les stratégies d’adaptation utilisées par les professionnels œuvrant en soins primaires afin de tendre vers une offre de soins et services bien adaptée et continue pour les patients atteints de plus d’une maladie chronique. Méthodes : Nous avons réalisé une étude de cas qualitative dans une organisation de soins primaires (groupe de médecine de famille – GMF au Québec). Nos données proviennent de deux sources : entrevues semi-structurées, documents. Notre analyse thématique des données a permis d’identifier les processus d’adaptation à l’œuvre. Résultats : Notre analyse permet d’identifier des défis posés par l’augmentation de la prévalence de patients souffrant de plus d’une maladie chronique et comment ils influencent la mise en œuvre de stratégies d’adaptation de la part des professionnels aux niveaux suivants : (1) les patients eux-mêmes, (2) les relations professionnels-patients, (3) les relations entre les professionnels du GMF, (4) les relations entre le GMF et les autres organisations de santé. L’exposition de ces phénomènes permet d’illustrer la dynamique d’émergence d’une forme d’organisation en réseau. Conclusion : Ce phénomène entraîne une transformation du cœur-même de l’organisation de l’offre de soins et services. Une compréhension plus approfondie de ses conditions d’émergence, de ses impacts et de sa gestion est requise.
