Abordagem da Nefropatia Diabética em Idade Pediátrica: Revisão Baseada na Evidência

Introdução: A diabetes mellitus é uma doença crónica com complicações macro e microvasculares que condicionam elevada morbilidade e mortalidade, destacando-se a nefropatia. O nosso objectivo foi rever a bibliografia existente sobre abordagem na nefropatia diabética em idade pediátrica. Métodos: Pesquisa nas bases UpToDate, National Guideline Clearinghouse, Guidelines Finder, Canadian Medical Association Practice Guidelines Infobase, Cochrane, DARE, Bandolier e PubMed, com limitação de busca para idades entre os 0 e 18 anos, de artigos em inglês, com os termos MeSH “Diabetes Mellitus”, “Diabetic Nephropathies” e “Renal Insufficiency”. Para atribuição da força de recomendação considerou-se a escala Strength of Recommendation Taxonomy (SORT) da American Family Physician. Resultados: Encontraram-se 167 artigos, dos quais se selecionaram 3 guidelines. Todas recomendam doseamento anual de microalbuminúria (grau B), não sendo consensual a idade de início. O controlo de factores de risco, como tensão arterial, variação glicémica e dislipidemia podem reduzir a progressão da lesão renal (grau B). O uso de IECAs ou ARAs pode diminuir a progressão da proteinúria (grau C). Discussão e Conclusão: O adequado controlo da diabetes mellitus e de factores de risco associados é recomendado para diminuição da progressão da nefropatia, e a sua monitorização deve ser feita regularmente com doseamento de proteinúria (grau B). Mais estudos são necessários para esclarecer a força de recomendação no uso de IECAs ou ARAs (grau C).
