Effectiveness of a Physical Activity Behavior Change Intervention for Breast Cancer Survivors: a Randomized Trial

Abstract
PURPOSE: To determine the effects of a physical activity behavior change intervention for breast cancer survivors on physical activity, fitness, muscle strength, and body composition. METHODS: Forty-one sedentary or insufficiently active breast cancer survivors currently taking aromatose inhibitors or estrogen receptor modulators were randomized to receive a 12 week physical activity behavior change intervention (n=21) or usual care (i.e., written materials only)(n=20). The intervention included discussion groups and supervised exercise sessions with gradual tapering to a home-based program with a post-intervention goal of 150 minutes of moderate walking per week. Measures were obtained at baseline and immediately post-intervention. RESULTS: Participants were primarily Caucasian (93%) with a mean age of 53 ± 9.2 and mean education of 14.7 ± 2.0 years. With regard to disease stage, 29% had stage I, 51% stage II, and 20% stage III. The intervention was well-liked and adherence to intervention activities was over 95%. Changes between post-intervention and baseline for the intervention versus the control group were assessed with independent t-tests. Significantly greater increases were noted for the intervention versus control group for accelerometer physical activity counts (+51744 versus -20359, p = .004). The intervention group also demonstrated a greater increase in fitness (i.e., estimated V02 from submaximal treadmill test = 4.9 versus 2.04 ml/kg/min, p = .058) and muscle strength (back/leg dynamometer = +8.15 versus -4.11 kilograms, p = .017). Although the intervention group demonstrated a significant decrease in the waist to hip ratio (-.03 versus .016, p = .018), no significant differences were noted for body mass index (-.29 versus -.37, p = .95) and percent body fat by dual x-ray absorptiometry (+.23 versus -.22, p = .50). CONCLUSIONS: A physical activity behavior change intervention for breast cancer survivors may have beneficial effects on activity behavior, muscle strength, fitness, and waist to hip ratio. Confirmation of intervention effectiveness and benefits in a larger phase III clinical trial is warranted.
