Abstract
Résumé Cette étude traite de l’évolution de programmes de gestion de la qualité dans des hôpitaux publics au Québec. Les résultats montrent que dans la presque totalité des sites examinés, l’introduction de la gestion de la qualité a répondu à des pressions institutionnelles de leur environnement plutôt qu’à des besoins internes. Or, celle-ci a surtout débouché sur la production de documents conformes aux exigences d’un organisme d’agrément mais sans qu’il y ait une incidence importante sur les valeurs et les pratiques de gestion dans ces hôpitaux. En revanche, dans au moins un des sites de l’étude, les principes de la gestion de la qualité semblent s’enraciner dans les pratiques organisationnelles. La vitalité de ce programme serait attribuable à la persistance d’initiatives visant la mise en œuvre d’activités de gestion de la qualité qui n’ont cependant pris leur envol qu’avec la mise en place d’une structure organisationnelle transfonctionnelle. Toutefois, les aménagements incrémentaux et les négociations qu’a aussi nécessité ce résultat indiquent qu’une telle expérience n’est que partiellement transposable à une autre organisation, dans un contexte d’adaptation mutuelle de la dynamique de l’organisation et des principes de l’approche qui y est introduite.
