Maligne periphere Nervenscheidentumore bei Neurofibromatose Typ 1 sind assoziiert mit erhöhter interner Tumorbelastung

Abstract
Patienten mit Neurofibromatose Typ 1 (NF1) entwickeln multiple benigne Nervenscheidentumoren und weisen ein 10%iges Lebenszeitrisiko auf, einen malignen peripheren Nervenscheidentumor (MPNST) zu entwickeln. Wir untersuchten 14 NF1 Patienten mit MPNSTs und 66 NF1 Patienten, die kein MPNST aufwiesen. Alle Patienten wurden detailliert klinisch untersucht und erhielten eine Ganzkörperkernspintomographie (GKK). Es wurde geprüft, ob die interne Tumorbelastung zwischen NF1 Patienten mit und ohne MPNST differiert. Das Durchschnittsalter der NF1 Patienten mit MPNSTs betrug 29,5 Jahre. Alle NF1 Patienten mit MPNST die jünger als 29,5 Jahre alt waren, zeigten interne plexiforme Neurofibrome, die sich mittels GKK zeigten. Das Volumen der Tumoren war bei diesen Patienten signifikant größer, als bei den NF1 Patienten im Alter jünger als 29,5 Jahre, die keine MPNSTs aufwiesen (p<0,02). Die NF1 MPNST Patienten jünger als 29,5 Jahre zeigten im Gegensatz zu den Betroffenen ohne MPNST im GKK mehr diffuse interne plexiforme Neurofibrome (p<0,05), mehr noduläre plexiforme Neurofibrome (p<0,05) und tiefe plexiforme Tumoren. Es fanden sich keine signifikanten Unterschiede im GKK im Hinblick auf die Tumorbildung zwischen NF1 Betroffenen mit und ohne MPNST, die älter als 29,5 Jahre alt waren. Die Ergebnisse der Studie deuten darauf hin, dass die Durchführung einer GKK bei jüngeren Patienten mit NF1, die frühzeitige Identifikation derjenigen Patieten erlaubt, die das höchste Risiko aufweisen ein MPNST zu entwickeln. Die engmaschige Überwachung dieser Risikopatienten mit NF1 könnte eine frühzeitige Diagnose und eine effektive Behandlung erlauben.
