Vollautomatische Segmentierung abdomineller CT-Angiographien – Technik und Evaluation

Abstract
Ziele: Ziel der Studie war es, ein Computerprogramm zu entwickeln und zu evaluieren, welches vollautomatisch, ohne jegliche Nutzerinteraktion, sämtliche knöchernen Strukturen aus abdominellen CT-Angiographien (CTA) entfernt und rotierende Maximum-Intensitätsprojektionen (MIP) zur Befundung und Demonstration erstellt. Methode: Das im Rahmen der Studie entwickelte Programm boneRemover erkennt automatisch knöcherne Strukturen in CTA-Datensätzen. Es entfernt diese Strukturen und erstellt rotierende MIPs. 30 aufeinanderfolgende abdominelle CTA-Untersuchungen wurden retrospektiv von drei Befundern bewertet. Im ersten Durchgang wurden mit einer modernen 3D-Workstation (Vitrea 3.7, Vital Images) semiautomatisch die Knochen segmentiert und standardisierte MIP-Rekonstruktionen angefertigt. Im zweiten Durchgang wurden die automatisch vom boneRemover erstellten Bilder analzsiert. In beiden Durchgängen wurde in definierten arteriellen Segmenten die Darstellungsqualität sowie die benötigte Zeit für die Nachverarbeitung und die Befundung der Bilder erfasst. Die Studie wurde von der Ethikkommission genehmigt. Ergebnis: Die manuelle Segmentierung konnte erfolgreich in 27 der 30 Datesätze (90%) durchgeführt werden. 24 (80%) der Fälle konnten erfolgreich vollautomatisch segmentiert werden. Die mittlere Darstellungsqualität der automatisch segmentierten Gefäße war höher als die der manuell segmentierten Gefäße (3,6 vs. 3,4; p<0,05). Die mediane Nachverarbeitungs- und Befundungszeit wurde durch den boneRemover von 9 auf 4min reduziert. Schlussfolgerung: Vollautomatische Segmentierung des Knochens ist mit dem beschriebenen Verfahren in der überwiegenden Anzahl der Fälle möglich und reduziert die Befundungszeit des Radiologen um 5min. Der Einsatz des boneRemovers spart so wertvolle Befundungszeit und produziert angiographieähnliche Bilder in diagnostischer Qualität.
