Laparoscopic Management of Ovarian Entrapment: A Rare Complication of IVF

Abstract
Abstract Background: Complications after oocyte retrievals are rarely reported. This report describes an unusual case of ovarian entrapment causing severe pelvic pain in a woman undergoing egg aspiration for in-vitro fertilization (IVF). Case: A 24-year-old female volunteering for egg donation experienced extreme pain after oocyte retrieval performed under anesthesia. Despite numerous attempts to provide analgesia, the pain remained to such an extreme extent that laparoscopy was performed to rule out torsion or other trauma. Laparoscopy revealed that the ovary had become displaced anterior to the uterus and was entrapped, but not torsed. Results: The patient's pain resolved once the ovary was placed back into the cul-de-sac . Conclusions: The overall procedural complication rate in women undergoing egg retrieval is low as is the risk of ovarian torsion. This is the first report of severe acute postretrieval pain caused by ovarian displacement; the report reinforces the need for careful examination of the ovaries when patients are under anesthesia.
