Abstract
Introduzione.In questo studio viene riportata l'incidenza e la resistenza alle penicilline (ox-res) di stafilococchi (staf) isolati da campioni clinici pervenuti in 7 microbiologie romane, appartenenti al gruppo di studio sulle Resistenze Batteriche nelle Infezioni Ospedaliere (SEERBIO).Materiali e Metodi.Gli antibiogrammi (ATB) sono stati eseguiti nel periodo 2001-02 su ceppi clinici tramite sistemi automatici e riuniti in unico database.Risultati.Sono stati analizzati 6754 staf.(23% su 28.830 ATB totali) di cui 3102 (46%) oxa-res.Aumento di staf del 2% nel 2002 ma con diminuzione degli oxa-res del 2% rispetto al 2001.Il maggior numero di Staf-oxa-res è stato isolato nelle rianimazioni (ICU, 69%) e nelle Chirurgie (61%) con diminuzione del 10% nel 2002.Tale riduzione riguarda anche: medicine -6%, ambulatori -1%, ematologie -62%, DH -1%.Il 16% degli staf di cui 17% oxa-res deriva da campioni ambulatoriali.Le ICU hanno l'incidenza maggiore di S.aureus Oxa-res (63% media, 68% 2001, 59% 2002), le chirurgie (48% media, 56% 2001, 42% 2002), le medicine (25% media), DH (16%), le ematologie (15%) e gli ambulatori (12%).Tra i coag.neg,S.epi.rappresenta il 55% del totale seguito da S.haemo.(15%) e S.hom.(8%).La oxares nei coag.neg.è variata nel periodo d'osservazione (55% nel 2001 e 40% nel 2002).Tra i coag.negisolati l'oxa-res è almeno del 35%, massima in S.xilosus (75%) seguito da S.haem.(62%) e S.epid.. e S.hom.(57%).Inoltre, si è notata la presenza di S.haem resistenti alla teicoplanina (12% nel 2001 e 16% nel 2002).Discussione.Accanto alla riduzione della oxa-res nel 2002 si è osservato che il 16% degli staf ambulatoriali è oxa-res, dimostrando una diffusione extraospedaliera del problema.Tuttavia, l'oxa-res nelle ICU e nelle chirurgie permane elevata.Infine l'incidenza di ceppi teico-resistenti può rendere il trattamento più complesso.
