Abstract
Wir berichten über eine 63-jährige Patientin, die 2 Jahre nach Diagnosestellung eines Cronkhite-Canada-Syndroms ein Adenokarzinom im Colon sigmoideum entwickelte und palliativ hemikolektomiert wurde. Das Operationspräparat wurde zur DNA-Sequenzierung und immunhistologischen Untersuchung aufbereitet und zeigte eine p53-Mutation ohne Veränderung des APC- und ras-Gens. Zusätzlich fanden sich Transkriptionsprodukte des erbB2-Protoonkogens. Das Cronkhite-Canada-Syndrom stellt eine nicht erbliche gastrointestinale Polyposiserkrankung dar, die überdurchschnittlich häufig zur Karzinombildung führt, obwohl die Polypen weder adenomatös noch hyperplastisch sind. Die Mutationsschritte scheinen nach der vorliegenden Analyse nicht der Adenom-Karzinom-Sequenz nach Vogelstein zu folgen, sondern einen unabhängigen Weg der Karzinogenese zu nehmen. Tumor association in cases of Cronkhite-Canada syndrome We report a 63-year-old lady with Cronkhite-Canada syndrome, who developed colorectal cancer. A hemicolectomy was performed, and the tumor specimen was prepared for DNA-analysis and immunohistochemical screening. We found a mutation of p53 gene without APC- and ras-gene alteration and expression of erbB2-protooncogen. The polyps in non-hereditary Cronkhite-Canada-syndrom are neither adenomatous nor hyperplastic, but patients often develop colorectal cancers. The steps of mutation do not follow the adenoma-carcinoma sequence, first described by Vogelstein 1988. This and previous observations suggest that carcinogenesis in Cronkhite-Canada syndrome follows another independent sequence.
