Abstract

Une enquête sur les télésoins infirmiers prodigués dans le cadre des programmes ambulatoires d’oncologie de l’Ontario a été effectuée de mai à juillet 2006. Sur les 38 programmes, 58 % utilisaient des infirmières en soins intégraux, 24 % utilisaient un système de triage centralisé et 18 %, un modèle mixte. Environ la moitié des programmes employaient des protocoles (61 %), assuraient la documentation systématique des appels (55 %), et/ou fournissaient, au personnel, une orientation aux télésoins infirmiers (45 %). Les programmes étaient peu nombreux (29 %) à surveiller la qualité des services de soins téléphoniques. Les bénéfices de cette pratique comprennent les suivantes : amélioration de l’accès des patients au système de santé; gestion précoce des symptômes; et continuité accrue des soins. Les obstacles dégagés en matière d’utilisation des protocoles étaient l’accès limité aux protocoles ou la faible sensibilisation à leur existence; la gestion de symptômes multiples; le manque de cohérence avec les pratiques des médecins; l’inquiétude que les protocoles prennent le pas sur le raisonnement critique; le manque de temps et le manque de protocoles électroniques. Les participants ont reconnu la nécessité d’avoir un système de documentation électronique, des critères normalisés pour la surveillance de la qualité des services et de nouvelles recherches en vue d’établir les points-repères des pratiques exemplaires.
