PROSPECTIVE INTERNATIONAL MULTICENTER EVALUATION OF THE SOFA,-APACHEII- AND MOF-SCORE IN PREDICTING MAJOR COMPLICATIONS AND OVERALL PROGNOSIS IN SEVERE ACUTE PANCREATITIS.

Abstract
Dynamic multiple parameter scoring systems such as the APACHEII-score are an established method for a fast and cost-effective severity assessment of acute pancreatitis (AP). However, their role in predicting major complications or overall prognosis has not been sufficiently validated yet. Patients and Methods: A total of 104 patients with severe AP were included in five European centers within 96 hours of disease onset. CT-proven intra-pancreatic necrosis was present in 72 (69%) patients, 17 (16%) patients developed infected necrosis (IN). Multiple organ dysfunction syndrome (MODS) was observed in 28 (27%) patients, 8 patients (8%) died. Sequential organ failure (SOFA)-, APACHEII-, and multiple organ failure (MOF/Goris)- sores were determined daily over a maximum of 21 days. Results: Receiver-operating characteristic (ROC) analysis was based on the two highest values of each scoring system within the overall observation period as well as on the respective scores at 48 hours after disease onset. The following results were obtained: The prediction of MODS and infected necrosis associated with MODS was possible with similar sensitivity and specificity rates. No statistical difference was found between the three scoring systems.TABLEConclusions: The APACHEII, SOFA, and MOF scores allow an early and reliable assessment of major complications as well as overall prognosis in patients with severe AP. Comprising fewer variables the SOFA- and MOF-score are easier to calculate and reach at least the same predictive performance.
