Introduction: Role of route of transmission on the duration of hepatitis B virus (HBV) infection is well known in children. The aim of our study is to assess the influence of HBV genotypes on HBe Antigen (Ag) loss. Methods: We retrospectively included 115 patients over a 19 year period. Clinical data were: sex, age at diagnosis and at the end-point, route of transmission (vertical or non-vertical), ethnic group, duration of HBS Ag infection and HBe Ag replication, influence of Interferon or Lamivudine therapy. Serological data were HBS Ag and HBe Ag status. After DNA extraction from frozen samples and PCR amplification, we performed genotype identification by Restriction Fragment Length Polymorphism. Results: Our series included 42 boys/73 girls. Contamination was at birth for vertical transmission (n=99) or at 4.5 year-old ± 3.2 SD (1 to 8.5) if non-vertical (n=7). Age at the end-point was 10.7.year-old ± 5.1 SD (0.5 to 21). They were from Asia (n=59), Africa (n=5), Europa (n=16), Canada (n=18), Latin or South America (n=11) and India (n=5). Sixty patients were HBe Ag negative and 5 HBS Ag negative. Eighteen patients were treated with Interferon and 6 with Lamivudine; remission was achieved respectively for 8 and 4 patients. Five genotypes were finded (A-E) within 106 patients, statistically associated with HBe Ag replication (chi2=10.55, p=0.03)(table 1). Kaplan-Meier survival analysis was significant for treatment (p<0.01), limit for genotype (p=0.05), ethnic group (p=0.06), route of transmission (p=0.07), but not significant for sex (p=0.84).Table 1: HBV genotype and HBe Ag statusConclusion: HBV genotype A is predominant within Hbe Ag negative group and genotype B in Hbe Ag positive group. Only treatment influences duration of replication. A larger cohort is needed to assess accurately the effect of genotype, ethnic group and route of transmission on HBV end of replication.
