The Role of Chlamydia Infection as a Risk Factor of HIV-1 Establishment and Spread in Heterosexual Populations

Abstract
Background and Rationale: Evidence of a causal link between classic STDs and HIV continue to accumulate. In countries where STD prevalence is relatively low and heterosexual HIV incidence is slowly but constantly increasing, the impact of STDs on HIV-1 transmission and its corresponding prevention potential remains uncertain. Objectives: To evaluate the role of chlamydia (Ct) infection on HIV establishment and spread in the strict heterosexual population (SHP) (1) in absence and (2) in presence of parenteral and homosexual transmission. To evaluate the potential of reducing STD prevalence to prevent HIV in the SHP. Methods: The analysis is based on a stochastic model of HIV and Ct transmission in a heterosexual population including various risk factors such as gender, age, sexual orientation, level of sexual activity, ethnicity, and drug use. The model includes the principal demographic, biologic, and behavioral characteristics of the diseases and the studied population. An initial parameter set (IPS) was chosen to represent the heterosexual population of Montréal-Center. A univariate and bivariate analysis was performed around IPS by varying both Ct prevalence and the magnitude of the Ct and HIV association (RRHIV/STD). The results obtained were generalized for a wide array of circumstances by varying simultaneously most parameter values based on Latin Hypercube sampling. Multivariate analysis was performed using regression and classification trees. Results: In a setting like Montréal-Center the probability of HIV establishment is 40% at IPS. Establishment and spread are very sensitive to Ct prevalence. In multivariate analysis Ct is the most influential parameter on establishment and spread. Conclusions: Since the risk of HIV establishment in the SHP is not nil, it is important to direct prevention efforts towards this population. Reduction of STD prevalence may prevent HIV establishment and spread in the SHP even in presence of constant reintroduction of HIV from bisexuals.
