Fardeau économique du paludisme sur les ménages démunis des quartiers défavorisés d’Abidjan, Côte d’Ivoire

Abstract
Les quartiers précaires d’Abidjan connaissent de graves problèmes d’assainissement exposant les habitants à d’importants risques sanitaires. Ces derniers doivent faire face quotidiennement au double poids des charges de maladie et de la pauvreté économique. Dans cette étude, 660 ménages ont été enquêtés dans 6 quartiers précaires et 3 villages lagunaires en vue d’évaluer leur vulnérabilité économique vis-à-vis du paludisme. Les résultats indiquent que les coûts directs associés au paludisme représentent en moyenne 12 à 14% du revenu des ménages dont les membres actifs perdent en outre 4 à 7 jours de travail. Le stress économique lié à cette maladie est d’autant plus important que les populations vivent avec moins d’un dollar US par habitant par jour couplé à une faible assistance de la part de la communauté. Les ménages adoptent des itinéraires thérapeutiques supposés à moindre coût mais ils ne tiennent pas compte du rapport coût efficacité de ces recours. Disadvantaged neighbourhoods in Abidjan have serious sanitation problems exposing inhabitants to numerous health hazards. Citizens are exposed to the burden of disease and economic poverty. In this study, 660 households were surveyed in 6 low-income neighbourhoods and 3 bay settlements in view of quantifying their economic vulnerability from malaria. The results indicate that direct costs associated with malaria represent 12 to 14% of household income. Active household members further lose 4 to 7 days of income due to the inability to work. The economic stress related to the disease substantial as the majority of people live with less than 1 US $ per day per inhabitant coupled to poor/insufficient mechanisms of community solidarity. The households opt for treatment itineraries at lowest cost without taking into account the cost effectiveness of the selected treatment schedule.
