A limitation of research investigating the effect of regular exercise on the development of breast cancer related lymphedema (BCRL) is that measures of arm circumference or volume have been used to indicate a change in status. Lymphoscintigraphy can provide semi-quantitative data on lymphatic function; however, no study has applied this technique to breast cancer survivors with and without BCRL during exercise. PURPOSE: Lymphoscintigraphy was used to measure changes in lymphatic function from rest to exercise in breast cancer survivors (BC, n=22) and breast cancer survivors with lymphedema (BCRL, n=10). METHODS: On separate days, subjects did either seated rest or the intermittent exercise protocol (12 repeated sets of arm cranking for 2.5 min at 0.6 W.kg-1 followed by 2.5 min of rest). 99mTc-antimony colloid was administered to the hands and 1 min spot views were taken with a γ-radiation camera immediately post-injection and every 10 min thereafter to allow for the calculation of clearance rate (CR). An upper body scan was also taken at 65 min post-injection to assess radiopharmaceutical uptake in the axilla (AX) and forearm (FORE) relative to the initial dose. RESUITS: CR significantly increased with exercise (p=0.00) in both groups (p=0.38). Neither group showed a significant change in AX with exercise (p=0.19) but BCRL had a significantly lower AX both at rest (1.4±1.2 vs. 4.5±3.3, p=0.05) and during exercise (1.7±2.1 vs. 5.8±3.6, p=0.03). FORE significantly increased from rest to exercise in BCRL only (2.4±0.87 vs. 4.4±1.9; p=0.00). Also, FORE was significantly higher in BCRL both at rest (2.4±0.87 vs. 1. 1±0.34, p=0.00) and during exercise (4.4±1.9vs. 1.1±0.25,p=0.00). CONCLUSIONS: Exercise results in similar increases in the clearance of radiopharmaceuticals from the hand with exercise in both groups, but does not increase the amount stored in axillary lymph nodes. FORE, which is used to indicate dermal backflow, did not change from rest to exercise in BC; however, there was a significant increase in FORE with exercise in BCRL. Taken with the CR and AX data, this would indicate the following: In women with BCRL, radiopharmaceuticals are able to clear from the hand at the same rate as BC with the addition of exercise, but instead of reaching the axilla, a greater amount of activity gets trapped in the dermis of the arm.
