Does Age Influence the VASIS II Classification Response to Head-up-Tilt Test?

Abstract
To determine the age-related VASIS II classification in patients with in patients with neurally mediated syncope (NMS) and positive head up tilt test (HUT). 97 subjects (M/F=43/54) with a clinical history of NMS and normal structural heart underwent HUT (70°) 30-minute protocol. 73 patients had positive HUT response (mean time to syncope of 19±7 min) and were included for study analysis. Patients were divided in three study groups for comparison according to the age: A = < 30 years (mean age 19±5); B = 31 to 65 years (50±10) and C = >66 years old (77±6). Beat-to-Beat heart rate (HR, bpm) and blood pressure were recorded using the Task Force Monitor (cnsystem, AU). Previous published VASIS II classification was used to characterized HUT responses (Type 1: Mixed response; Type 2a and 2b: Cardioinhibitory response and Type 3 vasodepressor response). Variables are shown as percentage of patients. Mixed responses during HUT are more prevalent in younger populations with NMS. Conversely, older patients have a higher prevalence of vasodepressor responses compared to younger patients. No cardiohinbitory responses were documented in older patients. Age related pathophysiological mechanisms during orthostatic stress may trigger specific vasovagal responses in susceptible individuals and should be taken into account when choosing therapeutic interventions.
