Wie zufrieden sind Brustkrebspatientinnen mit ihrer Behandlung?

Abstract

Fragestellung: Die Zufriedenheit von Patientinnen mit ihrer Versorgung gilt als Bestandteil der Behandlungsqualität. Ziel dieser Studie war, ein besseres Verständnis für die Bedürfnisse von Frauen mit Brustkrebs zu erlangen. Material und Methodik: Von 4538 Männern und Frauen mit verschiedenen Tumoren, die an der bundesweiten PASQOC®-Befragung zur Patientenzufriedenheit teilgenommen hatten, wurden die Subgruppen der 1132 Frauen mit Mammakarzinom und der 1485 Patientinnen mit anderen Tumoren separat analysiert. Die 99 verschiedenen Items des Fragebogens wurden zu 15 inhaltlichen Dimensionen zusammengefasst. Ausgewertet wurde die Problemhäufigkeit (PrH, prozentualer Anteil der Fragen mit als problematisch zu wertenden Antworten, idealer Wert 0 %). Ergebnisse: Die Brustkrebspatientinnen waren im Durchschnitt 59,9 Jahre alt und wurden überwiegend palliativ behandelt (46,4 %). Am zufriedensten waren sie mit den Dimensionen Schwestern/Pfleger und Arzt-Patienten-Beziehung (PrH 5,1 bzw. 10,6 %) sowie mit der „weiteren Unterstützung im Alltag“ (PrH 4,5 %). Dimensionen mit der geringsten Zufriedenheit waren Umgang mit Nebenwirkungen, Versorgung außerhalb der Praxis sowie Mitbestimmung/gemeinsame Entscheidungsfindung mit Problemhäufigkeiten von 41,4, 32,8 und 32,0 %. Diese Anteile lagen höher als bei Frauen mit anderen onkologischen Erkrankungen (PrH von 36,6, 27,1 bzw. 30,0 %). Schlussfolgerung: Brustkrebspatientinnen sind anspruchsvoller bzw. weniger zufrieden als Frauen mit anderen Tumorerkrankungen. Ärzte und Praxisteam sollten ihre Kommunikation stärker auf die Anforderungen dieser Patientinnen ausrichten und deren Familie und soziales Umfeld intensiver einbeziehen.
