PATHOGENESIS OF STAPH AUREUS EXIT SITE INFECTION IN PERITONEAL DIALYSIS

Abstract
We examined the pathogenesis of Staph.aureus (SA) exit site infection(ESI) in a rabbit model of peritoneal dialysis(PD) by using 3 different clinical strains of SA obtained from different PD patients.The virulence of SA strains varied from mild, moderarte and high levels.The purpose was to study the degree of ESI produced by the different strains of SA and their effect on peritonitis induced before and after PD process.PD catheters were introduced by midline abdominal incisonin NZW rabbits at Day 0,by injecting freshly prepared inoculum of different strains of SA at Day3, 5 and 7 at the catheter exit sites (1 x104 cfu/ml).to induce ESI. Six rabbits were used for each strain of SA, for a total of 18 rabbits.After innoculation, the rabbits were divided into 2 groups: Group A: dialysed by PD from Day 10 - 14; and GroupB: rabbits followed from Day 10–14 without dialysis. All rabbits were sacrificed at Day14.Tisuues from exit site, peritoneal cavity and PD fluids at necropsy were examined by histologic, microbiologic cultures and identification of the starins of SA. Results indicate that all dialysed and non dialysed rabbits developed significant ESI (GrA: 1.3±2.0x105cfu/gm; and Gr.B: 1.4± 2.7x105cfu/gm; (p= 0.035)irrespective of the virulence of SA strains used. Peritonitis, as detecetd from histologic and microbiological examinationof peritoneal tissues (cfu/gm)and fluids (cfu/ml), occurred in all dialysed rabbits(Group A) and in only 2 of 9 non dialyzed (Group B)rabbits (p=0.001).In conclusion, SA innoculation at exit site cause ESI irrespective of dialysis and virulence status of SA, but peritonitis from exit site is facilitated by PD process.
