Mesenteric‐reno‐caval shunt in an aged dog

Abstract
MADAM An unusual form of extrahepatic porto-systemic shunt was ultrasonographically diagnosed in an 11-year-old neutered male Pomeranian dog. On colour and pulsed Doppler ultrasound examination, the portal vein was normal in size with a normal flow appearance and velocity. No other ultrasonographic signs of portal hypertension were detected. Computed tomography angiography (CTA) was used to better characterise the porto-systemic shunt pattern and disclose a possible congenital or acquired nature. CTA images confirmed the presence of a large tortuous vessel originating from the caudal mesenteric vein and ending in the left renal vein (Fig 1). The left renal vein was increased in size compared to the right renal vein. The left gonadal vein was visible and within normal limits of size and appearance. Liver biopsies were performed and histopathology was consistent with glycogenic hepatopathy, moderate hepatic lipidosis and a slight degree of fibrosis. In a recent study, the conformation of 172 dogs with congenital porto-systemic shunts have been described, with colono-caval shunts reported for the first time (Fukushima et al. 2014). None of these dogs had a mesenteric-reno-caval shunt. On the basis of the clinical, imaging and histopathological findings, this case likely represents an atypical conformation of most likely congenital porto-systemic shunt in an old dog.
