Abstract
En el año 2006 se propuso una nueva definición (llamada de Montreal) de enfermedad por reflujo gastroesofágico (ERGE): "Afección que aparece cuando el paso del contenido del estómago hacia esófago produce síntomas molestos y/o complicaciones", es decir, el paso anormal del contenido gástrico (muy ácido), hacia el esófago, produce síntomas con o sin lesión inflamatoria de la capa interna del esófago (esofagitis).Los típicos de ERGE son: pirosis (sensación de quemazón o ardor en el pecho detrás del esternón) y/o regurgitación (sensación de retorno del contenido gástrico a faringe y boca).La ERGE es un problema de salud frecuente en el mundo desarrollado.Pirosis diaria la tienen alrededor del 3% de los adultos y el 30% la sufren al menos una vez al mes.La ERGE es benigna, pero los pacientes tienen peor calidad de vida, por sus síntomas, que la población general.Pueden aparecer síntomas acompañantes como náuseas, eructos, hipo, sialorrea (salivación excesiva), sensación de nudo (o globo) en garganta.Pero también pueden aparecer síntomas o signos de alarma, por ejemplo disfagia (dificultad al tragar por estrecheces en esófago), vómitos persistentes, anemia, hemorragia digestiva y pérdida de peso no intencionada.La ERGE es crónica, con intensidad y frecuencia variables y en los casos leves con periodos intermitentes de remisión más o menos largos.La pirosis y regurgitación en ausencia de síntomas y signos de alarma, se pueden tratar con antisecretores tales como inhibidores de la bomba de protones (IBP) como por ejemplo omeprazol, sin necesidad ineludible de pruebas complementarias (endoscopia, pH-metría, etc.) que reafirmen el diagnóstico.
