FMMP7 The Importance of Using a Sex-Based Analysis when Screening for Anxiety and Depression in Patients with Coronary Artery Disease

Abstract
Introduction: Having any type of heart event is clearly a stressful event. There is an increasing need to try to identify patients with CADwho would benefit from interventions to decrease psychological distress. The aim of this study was to determine the association between cortisol levels and the HADS scales among patients with CAD. Furthermore we wished to examine whether there were sex differences in the associations between cortisol levels and the HADS. Method: Eligible subjects included all adult Alberta residents over the age of 18 years, undergoing their first cardiac catheterization with 2 or more coronary arteries having ≥ 50% occlusion. The Outcome data were collected by means of a self-reported questionnaire mailed to patients within one week of their initial cardiac catheterization including 3 saliva tubes, a record sheet, instructions for collecting the saliva samples and the HADS questionnaire. Results: A convenience sample of seventy-one respondents returned the salivary samples. Among the women, there were significant negative correlations (elevated HADS scores were associated with lower cortisol levels) between the HADS anxiety score and the wake-up and 1/2 hour post wake-up cortisol levels. Conversely, among the males there were positive correlations (higher HADS scores were associated with higher cortisol levels) between the HADS anxiety scores and the cortisol levels, and statistically significant positive correlations between the HADS depression scores and all 3 salivary cortisol values.
