Resume Objectif : Cet article decrit l’experience du pharmacien dans le cadre du developpement et de l’implantation d’un suivi systematique pour la clientele ayant une maladie pulmonaire obstructive chronique a l’Hopital du Sacre-Coeur de Montreal. Mise en contexte : Le developpement d’un plan de gestion de l’episode de soins pour la clientele ayant une maladie pulmonaire obstructive chronique a debute en octobre 2004 a l’Hopital du Sacre-Coeur de Montreal. Un an plus tard, le suivi systematique de la maladie pulmonaire obstructive chronique a ete implante. Le pharmacien fait partie de l’equipe de soins qui est appelee dans tous les cas pour effectuer de l’enseignement, evaluer la pharmacotherapie et proposer des ajustements a la medication des patients participant a ce suivi. Jusqu’ici, l’evaluation de l’impact de ce projet semble demontrer une diminution de la duree moyenne de sejour des patients ayant une maladie pulmonaire obstructive chronique dans notre etablissement. Conclusion : Le pharmacien est un acteur cle dans le suivi systematique de la maladie pulmonaire obstructive chronique en etablissements de sante. Abstract Purpose: This article describes a pharmacist’s experience with the development and implementation of a systematic follow-up program for patients with chronic obstructive pulmonary disease at the Hopital du Sacre- Coeur, Montreal. Context: The development of a care plan for the episodic management of patients with chronic obstructive pulmonary disease began in October 2004 at the Hopital du Sacre-Coeur of Montreal. A year later, the systematic follow-up of chronic obstructive pulmonary disease was implemented. The pharmacist was part of the healthcare team that delivered education, evaluation of pharmacotherapy, and proposed dosage modifications for the medications used by this patient population. Up to now, results seem to show a decrease in the average number of days of hospitalization in our establishment for patients with chronic obstructive pulmonary disease. Conclusion: The pharmacist is a key player in the systematic follow-up of patients with chronic obstructive pulmonary disease in the hospital setting.
