Abstract
Resume Objectif : L’objectif de cet article est de decrire une experience pilote d’evaluation de la conformite des pratiques du circuit du medicament en oncologie au Centre hospitalier universitaire Sainte-Justine a partir d’une version de consultation du Guide sur les medicaments dangereux de l’Association pour la sante et la securite au travail – secteur des affaires sociales . Mise en contexte : Le Centre hospitalier universitaire Sainte-Justine est un etablissement mere-enfant de 500 lits, dont 32 en oncologie, 12 reserves aux cas de greffe de moelle osseuse et 20, au centre de jour. Notre equipe de recherche s’est interessee a la problematique des medicaments dangereux, notamment en raison de resultats locaux de contamination environnementale. Trois etudes de surveillance environnementale ont ete menees de 2004 a 2007. De plus, dans le cadre du projet Grandir en sante, un agrandissement du Centre de cancerologie Charles-Bruneau a ete acheve au printemps 2007 avec une nouvelle pharmacie satellite de 125 m2 inauguree en juin 2007. La planification de cette nouvelle pharmacie a ete effectuee a la lumiere des travaux du comite sur les medicaments dangereux de l’Association pour la sante et la securite au travail – secteur des affaires sociales. Une evaluation de la conformite des pratiques touchant strictement le Depar tement de pharmacie avait ete menee prealablement au regard du chapitre USP-797 et de l’alerte NIOSH. Conclusion : Il s’agit de la premiere evaluation de conformite menee a partir du Guide sur les medicaments dangereux de cette association. A partir des 218 criteres cotes, on observe une proportion de criteres non conformes s’elevant a 30 %, de criteres partiellement conformes, a 37 % et de criteres conformes, a 30 %. Une evaluation de la conformite des pratiques du circuit du medicament en oncologie est une demarche pratique et applicable, qui peut mener a une mise a niveau de la pratique. Abstract Objective : The purpose of this article is to describe a pilot experience with compliance evaluation of the oncology drug-handling practices at the Centre hospitalier universitaire Sainte-Justine, using a preliminary version of the Guide sur les medicaments dangereux, published by the association for health and safety in the workplace, social affairs sector. Context : The Centre hospitalier universitaire Sainte- Justine is a 500-bed maternal-child hospital, of which 32 beds are for oncology, 12 beds reserved for bone-marrow transplants, and 20 beds for the day centre. Our research team was interested in the issue of hazardous drugs as a result of local environmental contamination tests. Three studies on environmental surveillance were undertaken from 2004 to 2007. Furthermore, within the context of the Grandir en sante project, renovations to the Charles Bruneau cancer centre were finalized in spring of 2007, with the addition of a new 125 m2 satellite pharmacy that was inaugurated in June of 2007. The plans for this new pharmacy were developed in light of the work done by the committee on hazardous drugs from the association for health and safety in the workplace, social affairs sector. A compliance evaluation of the practices affecting only the pharmacy department had been previously done in response to chapter USP-797 and the NIOSH alert. Conclusion : This is the first compliance evaluation done using the association’s Guide sur les medicaments dangereux. According to the 218 established criteria, the hospital drug-handling practices did not comply with 30% of criteria, were partially compliant with 37% of criteria, and were in compliance with 30% criteria. An evaluation of the compliance of the oncology drug-handling practices is practical and applicable and can lead to an update in standards of practice.
