Nous avons peu de connaissance sur les traitements standard du cancer ovarien chez les femmes âgées, en termes de faisabilité, de tolérance et d’efficacité, du fait de leur relative sous-représentation dans des essais cliniques. Le traitement standard du cancer ovarien est la chirurgie maximale suivie de chimiothérapie. Cependant, ces standards reposent sur des essais cliniques incluant peu de patientes âgées. Par exemple, l’intérêt des taxanes en association aux sels de platine n’est pas encore clairement précisé. Il faut tenir compte de la vulnérabilité des personnes âgées, afin de savoir quelles chimiothérapie et chirurgie exécuter et pour quelles patientes, en combinant, comme dans l’étude PACE par exemple, des outils d’évaluation des risques chirurgicaux avec les composants de l’évaluation gériatrique (EG). Les données aujourd’hui disponibles permettent de distinguer les patientes en situation de vieillissement réussi et les patientes vulnérables et de poser pour chaque souspopulation les questions les plus pertinentes dans le cadre d’essais randomisés internationaux.
