Abstract
Il est admis que 70% des résidents vivant en EHPAD sont déments. Les patients ayant une maladie d’Alzheimer sont, comme beaucoup de malades âgés, particulièrement exposés aux psychotropes, et en particulier aux neuroleptiques. Si cette étude rétrospective a considéré beaucoup de paramètres, elle a néanmoins permis de mettre en lumière quelques résultats importants. Sur les 379 lits que compte son unité des soins de longue durée, l’Institut Universitaire de Gériatrie de Montréal (IUGM) héberge 36% de patients dont le traitement comporte ou a comporté des antipsychotiques atypiques (APA). 28% d’entre eux n’avaient pas de traitement à leur admission et seulement 1 patient sur 4 a vu son traitement d’APA stoppé. 13,1% des patients ont un traitement antipsychotique prescrit « à vie » (date d’expiration de l’ordonnance en 2027). Seuls 18,1% des patients reçoivent un APA pour démence. La majorité des APA (62,7%) ont été prescrits pour des troubles du comportement. La rispéridone et la quétiapine sont de loin les principes actifs les plus prescrits à l’Institut (elles concernent respectivement 43,1% et 39,4% des patients).
