Les auteurs canadiens ont compare pres de 20 000 patients atteints de maladie d’Alzheimer traites par inhibiteurs de la cholinesterase a 60 000 temoins non traites Dans cette cohorte, les hospitalisations pour syncope ont ete plus frequentes chez les patients sous anticholinesterases (donepezil : 13 641 patients, galantamine : 3 448, rivastigmine : 2 714) que chez les castemoins (31,5 vs 18,6 evenements pour 1 000 personnes-annees : HR 1,76 ; 1,57-1,98). Il en a ete de meme pour les consequences de ces syncopes : bradycardie (HR 1,69 ; 1,32-2,15), bloc auriculo-ventriculaire complet necessitant l’implantation d’un pacemaker (HR 1,49 ; 1,12-2,00) et fractures de hanche (HR 1,18 ; 1,04-1,34). Ces resultats persistent quelles que soient les modalites choisies pour les couples patients/temoins. L’utilisation des anticholinesterases est donc bien associee a un taux accru d’evenements graves chez ces patients de plus de 68 ans atteints de maladie d’Alzheimer. Ce risque, jusqu’a maintenant sous-estime, doit donc etre soigneusement pris en compte avant toute prescription de ces medicaments dont les avantages sont au mieux tres modestes… Gill SS, Anderson GM, Fischer HD, Bell CM, Li P, Normand SLT, Rochon PA. Les questions que se pose la redaction • Cette etude canadienne sur une periode de 2 ans (2002-2004) a bien sur les limites d’une etude observationnelle et retrospective. Il s’agit cependant d’une vaste cohorte dont les resultats sont statistiquement et cliniquement significatifs. • La vulnerabilite de ces patients devrait logiquement conduire a une extreme prudence – contraire, il est vrai, aux « recommandations » actuelles – dans l’utilisation de ces medicaments. Ne parlons plus « d’effet structurant » de la prescription quand elle devient dangereuse !
