Abstract
Une proctectomie totale avec anastomose coloanale a été réalisée chez 65 patients pour cancer du bas rectum. Chez 20 patients, un réservoir colique a été créé, les 45 autres ayant une anastomose directe. La morbidité et la mortalité postopératoires ont été nulles dans les deux groupes. Le résultat fonctionnel a été apprécié par un examen clinique et une manométrie anorectale. La fréquence des selles a été inversement proportionnelle au volume maximum toléré du segment colique abaissé (p < 0,001). Pendant la première année, 60 % des patients avec réservoir et 33 % des patients sans réservoir n’avaient qu’une selle par jour (p < 0,01). Le volume maximal toléré était augmenté dans le groupe réservoir (p < 0,05). L’ablation du rectum et donc la perte de la capacité de réservoir de celui-ci augmente la fréquence des selles lors de la réalisation d’une anastomose coloanale. La création d’un réservoir colique améliore le résultat fonctionnel en augmentant le volume maximal toléré sans augmentation de la morbimortalité
