Unusual cardiac presentation in neurofibromatosis type 1

A 21-year-old man was evaluated due to past history of recurrent ventricular tachycardia in infancy in the setting of previously documented neurofibromatosis type 1. Physical examination was remarkable for below-knee amputation undertaken for a painful tibial tumour. Electrocardiogram revealed anterior leads T-wave inversion. Transthoracic echocardiography showed possibly two echobright masses embedded within the ventricular septal myocardium and extending towards the left ventricular (LV) apex (Panel A, arrows; Supplementary data online, Movie S1) with no evidence of flow within the masses by colour Doppler. The remarkable size of the masses was best demonstrated by cardiac magnetic resonance (CMR). A large heterogeneous hypointense mass measuring (5.1 × 7.4 × 6.0 cm) was noted on cine imaging within the ventricular septum extending into both ventricular apices, suggestive of fibrous tumour, with no evidence of inflow/outflow obstruction of either ventricle (Panel B, arrow; Supplementary data online, Movies S2–4). Dynamic gadolinium contrast-enhanced CMR perfusion imaging demonstrated a non-contrast enhancing mass encircled by a thin layer of normally perfused myocardium (Supplementary data online, Movie S5). Delayed enhancement images illustrated intense enhancement of two masses separated by a thin wall of non-enhancing normal myocardium (Panel C, arrow) consistent with two septal fibrous tumours.
