Auteur(s) : Jean-Philippe Spano, Jean-Jacques Mazeron Le traitement standard du glioblastome repose sur une chirurgie radicale, lorsqu’elle est possible, suivie d’une radiotherapie, avec ou sans chimiotherapie adjuvante. Le pronostic est globalement defavorable. Les durees medianes de survie sont dans la plupart des series de 9 a 12 mois, et le taux de survie a 2 ans de 8 % a 12 % [1]. Le temozolomide, agent alkylant, a fait la preuve de son activite dans les gliomes en rechute [2, 3]. L’etude randomisee de phase II de Yung et al. [4], qui a compare le temozolomide a la procarbazine chez des patients atteints de recidive de glioblastome, a conclu a la superiorite du temozolomide en termes de taux de survie a 6 mois (21 % contre 8 %, p < 0,008). En se fondant sur des resultats experimentaux laissant augurer une synergie entre le temozolomide et l’irradiation in vitro, Stupp et al. [5], dans une etude recente de phase II, ont aussi montre, chez 64 patients atteints de glioblastome, que la chimioradiotherapie concomitante avec temozolomide non seulement etait bien toleree mais pouvait aussi etre efficace. La duree mediane de survie etait de 16 mois et les probabilites de survie a 1 et 2 ans etaient respectivement de 58 % et 31 % ; les patients de moins de 50 ans et ceux ayant beneficie d’une exerese radicale avaient les meilleurs resultats. Il semblait donc legitime de mettre en route une etude de phase III pour confirmer ces resultats. Elle a ete realisee par l’EORTC (European Organization for Research and Treatment of Cancer) et le NCIC (National Cancer Institute of Canada). Les premiers resultats ont ete presentes en session pleniere au congres annuel de l’Asco (American Association of Clinical Oncology) de 2004.  L’essai a inclus 573 patients issus de 85 centres. Les patients, âges de 18 a 70 ans (mediane 56 ans), etaient tous atteints d’un glioblastome nouvellement diagnostique sur la base d’un prelevement anatomopathologique. L’indice de performance etait selon l’OMS de 0 a 2. Quarante-cinq pour cent ont beneficie d’une exerese macroscopiquement complete et 44,5 % d’une exerese partielle. Tous les patients ont alors recu moins de 6 semaines plus tard une irradiation focale de 60 Gy en 30 fractions et 6 semaines. La moitie ont recu, apres randomisation, une chimiotherapie par le temozolomide. La chimiotherapie a debute en meme temps que la radiotherapie. La prise de temozolomide etait quotidienne, a la dose de 75 mg/m2, samedi et dimanche compris, pendant les 6 semaines de la radiotherapie. La chimioradiotherapie concomitante etait suivie de 6 cycles de 5 jours de temozolomide debutant toutes les 4 semaines, a la dose de 150 a 200 mg/m2/j.  La dose de radiotherapie a pu etre delivree a 93 % des patients. La chimiotherapie concomitante a ete administree sans interruption a 76 %, avec interruption temporaire chez 11 % ; elle a du etre interrompue prematurement chez 12 %. La chimiotherapie adjuvante a ete donnee a 76 % des patients, mais seulement 36 % ont recu les 6 cycles. La duree mediane de suivi etait de 2 ans. Les durees medianes de survie sans progression tumorale etaient respectivement de 7,2 et 5 mois dans les groupes avec et sans temozolomide (p < 0,001), celles de survie globale de 15 et 12 mois (p < 0,001). Les probabilites de survie a 2 ans etaient de 26 % et 8 % (p < 0,001). Une toxicite hematologique de grade 3 ou 4 a ete observee chez 7 % des patients pendant la chimioradiotherapie concomitante et chez 16 % pendant la chimiotherapie adjuvante. Au total, il s’agit de la premiere etude qui demontre ainsi un avantage nettement significatif en termes de survie pour des patients atteints de glioblastome multiforme traites par chimiotherapie. L’identification de nouveaux marqueurs biologiques predictifs de reponse est egalement en cours. Il faudra bien sur attendre les analyses ulterieures pour se faire une opinion definitive de l’association de radiotherapie et de temozolomide chez les patients atteints de glioblastome, mais beaucoup considerent deja qu’il s’agit la du nouveau protocole standard international. Il s’agit en tout cas du seul progres significatif obtenu dans le traitement de cette maladie de pronostic defavorable depuis bien longtemps. On peut cependant s’interroger sur le role respectif des chimiotherapies concomitante et adjuvante dans l’amelioration de la probabilite de survie. References 1. Levin V, Leibel S, Gutin P.
