P05.07: Early pregnancy serum biochemical markers and advanced placental maturation

Abstract
To evaluate the spectrum of serum biochemical markers in pregnancies identified with syncytial knotting (SK), a sign of advanced placental maturation. All placentas at our institution undergo pathological evaluation after delivery. In 2012, 62 placentas with SK were identified. The serum biochemical parameters results of these cases were obtained through the provincial aneuploidy screening program, evaluated and compared to 61 randomly selected gestational age-matched controls with no pathological abnormality. Statistical analysis used the Wilcoxon test for non-parametric parameters, the t-test for normally distributed values, logistic and linear regression. The mean birthweight centile was lower in cases with SK (mean 46%, interquartile range 31–75%) compared to those without (mean 54%, interquartile range 17–74%), although this difference did not achieve significance (p = 0.14). In pregnancies with SK, the PAPP-A was significantly lower in pregnancies with male rather than female fetuses (p = 0.0007). Linear regression of the effect of aneuploidy screening parameters (biochemical markers and maternal age) demonstrated that elevated AFP and young maternal age were associated with earlier gestation at delivery (AFP, p = 0.004, age p = 0.007) with elevated inhibin-A tending towards earlier gestation (p = 0.074). Elevated AFP was also associated with a lower birthweight centile (p = 0.013). Elevated AFP is known to be associated with adverse pregnancy outcomes in the absence of other abnormalities. In our population this was associated with lower birthweight centile and gestational age at delivery in association with histological evidence of advanced placental maturation. Male fetuses with SK also demonstrated lower PAPP-A levels than females. Histological examination of the placenta after birth may help to corroborate the pathophysiology behind some of the adverse outcomes associated with abnormal serum biochemical markers in pregnancy.
